В исследование было включено 203 больных с опухолью Клацкина (основная группа) и 191 пациент без билиарной патологии (контрольная группа). Всем больным была проведена МРТ брюшной полости с МР-холангиопанкреатографией (МРХПГ). Кроме того, 141 больному гилюсной холангиокарциномой была выполнена прямая холангиография. Конфлюенсы желчных протоков типировались по классификации Накамура.</p></sec><sec><title>Результаты</title><p>Результаты. В основной группе конфлюенсы I, II, III, IV и V типов по Накамура встречались в 27,6; 22,2; 1,5; 36,0 и 2,4 % случаев соответственно; в 10,3 % наблюдений типировать конфлюенс не удалось. В контрольной группе распределение типов конфлюенса желчных протоков в целом соответствовало общепопуляционному: I, II, III, IV и V типы были выявлены у 75,0; 9,3; 0,5; 14,2 и 1,0 % больных соответственно. Различия между группами были значимыми (p&lt;0,001). Предложены статистическая и гидродинамическая модели конфлюенсов, объясняющие более высокую частоту развития гилюсной холангиокарциномы при нестандартных типах слияния желчных протоков.</p></sec><sec><title>Заключение</title><p>Заключение. Частота встречаемости нетипичных конфлюенсов желчных протоков у больных опухолью Клацкина значительно превышает таковую у больных без билиарной патологии, что может быть обусловлено повышенным внутрипротоковым давлением и большей площадью стенок.</p></sec></abstract><trans-abstract xml:lang="en"><p>The purpose of the study was to assess the frequency of bile duct confluence variations in patients with hilar cholangiocarcinoma and in patients with no biliary lesions.</p><sec><title>Material and Methods</title><p>Material and Methods. The study included 203 patients with Klatskin tumor (study group) and 191patients having no bile duct lesions (control group). All patients underwent abdominal MRI including MR-cholangiopancreatography. In addition, 141 patients from the study group underwent percutaneous transhepatic cholangiography. Anatomic variations of the bile duct confluence were based on Nakamura classification.</p></sec><sec><title>Results</title><p>Results. In the study group, type I, II, III, IV and V confluences were detected in 27.6 %, 22.2 %, 1.5 %, 36.0 % and 2.4 % of cases, respectively. In the control group, the frequencies of each type of the bile duct confluence were as follows: type I in 75.0 %, type II in 9.3 %, type III in 0.5 %, type IV in 14.2 % and type V in 1.0 %. Differences between groups were statistically significant (p&lt;0.001). Statistical and hydrodynamic bile duct confluence models that could explain the differences between the groups were proposed.</p></sec><sec><title>Conclusion</title><p>Conclusion.
