Унифицированного подхода к ведению пациентов с меланомой хориоидеи (МХ) малых размеров (толщина до 3 мм, основание до 10 мм) не существует. Изучение развития метастатической болезни у данной категории пациентов является актуальным для выбора метода лечения.</p><p>Цель исследования – оценить частоту развития метастатической болезни у пациентов с меланомой хориоидеи малых размеров, пролеченных с использованием транспупиллярной термотерапии (ТТТ), фотодинамической терапии (ФДТ), брахитерапии (БТ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В ретроспективное исследование включены сведения о 149 пациентах с меланомой хориоидеи, пролеченных в РНПЦ онкологии и медицинской радиологии им. Н.Н. Александрова с 2005 по 2018 г. Все включенные в исследование пациенты имели опухоли менее 10 мм в диаметре, менее 3 мм толщиной и не имели признаков системного прогрессирования до начала терапии. Все опухоли соответствовали стадии Т1N0M0 (American Joint Committee on Cancer (AJCC)). С использованием ФДТ пролечено 44, с использованием ТТТ – 47, с использованием БТ – 58 пациентов.</p></sec><sec><title>Результаты</title><p>Результаты. Медиана наблюдения после брахитерапии составила 154 мес (12 лет), после ТТТ – 128 мес (10 лет), после ФДТ – 72 мес (6 лет). За период наблюдения системное прогрессирование было зарегистрировано у 1 (2,3 %) пациента после ФДТ и у 5 (10,6%) пациентов после ТТТ. У пациентов после БТ системного прогрессирования за период наблюдения выявлено не было. Все случаи развития метастатической болезни были ассоциированы с местным рецидивом или продолженным ростом МХ.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. При применении лазерных методов 5-летняя безметастатическая выживаемость после ТТТ оказалась хуже, чем после ФДТ (82 ± 8,0 % и 94 ± 6,0 % соответственно, р&lt;0,001). При этом в клинической практике в ряде случаев предпочтение может отдаваться лазерным методам лечения, позволяющим избежать постлучевых ретинопатий, так как для высокого качества жизни важное значение имеет сохранение остроты зрения. Отсутствие локального контроля опухоли можно считать суррогатным маркером развития метастатической болезни. заключение. There is no unified approach to the management of patients with small choroid melanoma (CM) (thickness up to 3 mm, base diameter up to 10 mm). The study of the development of metastases in these patients is of great significance for choosing an appropriate treatment method.</p></sec><sec><title>Purpose</title><p>Purpose: to assess the incidence of metastatic disease in patients with small CM, who were treated with transpupillary thermotherapy (TTT), photodynamic therapy (PDT), and brachytherapy (BT).</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The retrospective study included 149 patients with CM, who were treated at the National Cancer Center of Belarus from 2005 to 2018. All patients had tumors less than 10 mm in diameter, less than 3 mm in thickness, and had no signs of systemic progression before starting therapy. All tumors corresponded to stage T1N0M0 (American Joint Committee on Cancer (AJCC)). 44 patients were treated with PDT, 47 with TTT, and 58 with BT.</p></sec><sec><title>Results</title><p>Results. The median follow-up time was 154 months (12 years) in patients treated with brachytherapy, 128 months (10 years) in patients treated with TTT and 72 months (6 years) in patients treated with PDT. During the follow-up period, metastases were observed in 1 (2.3 %) patient after PDT and in 5 (10.6 %) patients after TTT. In patients treated with BT, systemic progression was not recorded during the follow-up period. All cases of metastatic disease were associated with local recurrence or continued growth of CM.</p></sec><sec><title>Discussion</title><p>Discussion.
