Белку Не-4 в последнее время уделяется большое внимание из-за его диагностической и прогностической ценности при эпителиальном раке яичников, что обусловлено его патогномонической ролью при этом заболевании. Еще одним значимым патогенетическим фактором является фактор роста эндотелия сосудов (VegF), который играет ключевую роль в неоангиогенезе. Цель исследования – изучение содержания Не-4 и VegF-a в опухолевой и неизмененной ткани яичника, сальнике и брюшине до и после неодъювантной химиотерапии с включением препаратов платины. Материал и методы. В исследование включены 93 больных раком яичников t2–3Nxm0–1 стадии, получивших хирургическое и химиотерапевтическое лечение в ФГБУ «НМИЦ онкологии» МЗ РФ, г. Ростова-на-Дону, из них у 42 больных лечение начато с неоадъювантной химиотерапии, за которой последовали операция и послеоперационная цитостатическая терапия; 51 пациентке лечение начинали с хирургического вмешательства, затем проводили химиотерапию. В тканях больных раком яичников определяли уровень Не-4 и VegF-a. В качестве контроля с целью определения референтных значений Не-4 и VegF-a в тканях явились образцы тканей 17 больных, прооперированных по поводу доброкачественной патологии. Результаты. Проведено сравнение между группами больных, получавших и не получавших неоадъювантную терапию, а также в группах больных в зависимости от эффективности цитостатического лечения. Оценку эффекта проводили через каждые 3 курса химиотерапии, используя шкалу Recist 1.1. Показано, что уровень Не-4 в первичных и метастатических тканях, пораженных и не пораженных опухолью, у больных раком яичников исходно повышен. При этом эффективность химиотерапевтического лечения напрямую связана с уровнем данного показателя, не изменяясь либо повышаясь при нечувствительных к лекарственному лечению опухолях. Уровень же VegF-a значимо отличался в тканях, пораженных и не пораженных опухолью, что свидетельствует о значимом его патогномоничном эффекте не «до», а на этапах морфологической малигнизации. Динамика снижения VegF-a в данном исследовании не зависела от эффекта химиотерапии. Заключение. Маркер Не-4 – патогномоничный фактор развития рака яичников, предшествующий морфологическим признакам злокачественности и отражающий эффективность химиотерапии, тогда как VegF-a является, скорее всего, следствием развития злокачественного процесса.</p></sec></abstract><trans-abstract xml:lang="en"><p>Introduction. Recently, the he-4 protein has received great attention due to its diagnostic and prognostic abilities in epithelial ovarian cancer. In addition to its diagnostic value, this protein is involved in the pathogenesis of ovarian cancer. Another significant pathogenetic factor is the vascular endothelial growth factor (vegf) which plays a key role in neoangiogenesis. The purpose of the study focused on the analysis of he-4 and vegf-a levels in tissues of ovarian cancer, in healthy contralateral ovaries and in common metastatic tumors in the omentum and peritoneum to determine the place and role of these tumor markers at the stages of carcinogenesis. Material and methods. The study was performed using the abovementioned tissues of 93 patients with t2-3nхm0-1 ovarian cancer. 51 patients underwent surgery followed by chemotherapy. 42 patients received initial neoadjuvant chemotherapy followed by surgery and adjuvant cytostatic therapy. Tissue samples from 17 patients with benign diseases were used as the control for determining the reference values for he-4 and vegf-a. A comparison was made between groups of patients with and without neoadjuvant therapy, as well as in groups of patients depending on the effectiveness of cytostatic treatment. Results. The levels of he-4 in primary and metastatic tissues affected and not affected by cancer were initially elevated in patients with ovarian cancer. The chemotherapy effectiveness directly correlated with the level of he-4 reduction, which did not change or increased in tumors resistant to medical treatment. The level of vegf-a significantly differed in cancer and non-cancer tissues, which indicated its significant pathogenetic effect not “before”, but at the stages of morphological malignization. The dynamics of vegf-a decrease in this study did not depend on the chemotherapy effect. Conclusion.
