Рак желудка занимает одно из первых мест по заболеваемости в России и мире среди всех злокачественных новообразований. В 40 % случаев постановка диагноза происходит лишь на поздних стадиях развития – от Т3 и выше, что ведет к увеличению риска отдаленного метастазирования. Перитонеальный канцероматоз при раке желудка является одним из самых частых путей диссемининации с медианой выживаемости не более 6 мес.</p><p>Цель исследования – изучить различные современные подходы к лечению перитонеального канцероматоза при раке желудка.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Поиск литературы производился в системах Medline, Cochrane Library, E-library, Scopus, PubMed.</p></sec><sec><title>Результаты</title><p>Результаты. Существует большое разнообразие комбинированных подходов к лечению метастатического рака желудка. Разрабатываются различные варианты циторедуктивных операций, дополненных неоадъювантной и адъювантной химиотерапией, внутрибрюшным введением цитостатиков, лучевой терапией. Однако результаты исследований в вопросе увеличения выживаемости и снижения рецидивов при диссеминированном процессе противоречивы. Наиболее неоднозначную группу при выборе тактики лечения представляют пациенты, у которых морфологически и цитологически доказано наличие свободных опухолевых клеток в перитонеальном смыве с брюшной полости при отсутствии визуализируемых очагов внутрибрюшного метастатического поражения. Рецидив рака желудка в данной группе развивается в течение 2 лет. Кроме того, показано, что 5-летняя выживаемость у пациентов с наличием макроскопических очагов и с выявленными свободными опухолевыми диссеминатами в смывных водах сопоставима и составляет 2 %. Одной из наиболее эффективных экспериментальных тактик лечения перитонеального канцероматоза при раке желудка и яичников являлась внутрибрюшная радионуклидная терапия с применением коллоидных радиофармпрепаратов на основе 198Au. Основным достоинством метода являлось полное прекращение формирования выпота в брюшную полость при асцитных формах заболевания. Однако из-за высокой кишечной токсичности препаратов радиоактивного золота и лучевой нагрузки на пациентов и медицинский персонал дальнейшая работа в этом направлении была остановлена.</p></sec><sec><title>Заключение</title><p>Заключение. На настоящий момент продолжается поиск наиболее эффективной тактики лечения больных с перитонеальным канцероматозом при раке желудка, направленной на улучшение выживаемости.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Gastric cancer (GS ) is one of the most common and deadly cancers in Russia and worldwide. In 40 % of cases, GC is diagnosed at an advanced stage, thus increasing the risk of distant metastasis. Peritoneal carcinomatosis from GC is one of the most common pathways of dissemination, with a median survival time of less than 6 months.</p></sec><sec><title>Aim</title><p>Aim. To study various modern approaches to the treatment of peritoneal carcinomatosis in gastric cancer.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. Literature search was performed in Medline, Cochrane Library, Elibrary, Scopus, PubMed systems.</p></sec><sec><title>Results</title><p>Results. Currently, there is a wide variety of approaches to combined modality treatment of metastatic GC . Various options of cytoreductive surgeries are being developed. These surgeries are combined with neoadjuvant/adjuvant, intra-abdominal chemotherapy and radiation therapy. However, the results of studies on improving survival and reducing recurrence in patients with advanced GC are contradictory. Currently, patients with morphologically and cytologically confirmed free cancer cells in the peritoneal lavage without visualized intra-abdominal metastatic lesions are the most controversial group for the choice of appropriate treatment. Gastric cancer recurrence in these patients occurs within 2 years. In addition, the 5-year survival rate in patients with the presence of free cancer cells in peritoneal washings amounts for 2 %. One of the most effective experimental treatments for peritoneal carcinomatosis in gastric and ovarian cancers was intra-abdominal radionuclide therapy using colloidal198Au. The main advantage of the method was the complete cessation of the formation of effusion into the abdominal cavity in ascites forms of the disease. However, due to the high intestinal toxicity of radioactive gold tracer and radiation exposure to patients and medical staff, further work was stopped.</p></sec><sec><title>Conclusion</title><p>Conclusion.
