Реконструктивная хирургия головы и шеи при злокачественных опухолях представляет собой один из сложных разделов в онкологии и требует от специалиста владения и знания различных методов замещения дефектов. Выбор варианта реконструкции определяется различными факторами: размером, локализацией и составом дефекта, возрастом, сопутствующей патологией, прогнозом заболевания, предпочтениями хирурга и пациента. В арсенале хирурга довольно широкий выбор пластического материала – от свободного расщепленного до сложных по составу реваскуляризированных лоскутов. Лидирующим методом в лечении больных со сложными дефектами органов головы и шеи является микрохирургия. Однако использование реваскуляризированных аутотрансплантатов возможно не у всех категорий больных, в связи с чем продолжается поиск новых методов реконструкции с целью улучшения функциональных, эстетических результатов и снижения травматичности.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Представлен анализ использования различных вариантов слизисто-мышечного лоскута на лицевых сосудах для реконструкции дефектов 7 первичных пациентов с морфологически верифицированным раком слизистой оболочки полости рта Т2–4N0–1 стадии, которые получали лечение в НМИЦ онкологии им.Н.Н. Блохина в период с июня 2020 г. по март 2021 г. На первом этапе комбинированного лечения выполнялось хирургическое вмешательство с одномоментной реконструкцией различными вариантами слизисто-мышечного лоскута на лицевых сосудах в плане. Всем пациентам была проведена адъювантная лучевая терапия.</p></sec><sec><title>Результаты</title><p>Результаты. При выкраивании лоскута не выполняется дополнительного кожного разреза, дефект в полости рта замещается аналогичными по составу тканями с сохранением малых слюнных желез, соответственно, отсутствует рост волос и дефект донорского ложа. Ни в одном случае не было нарушения питания лоскута.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные хорошие функциональные и эстетические результаты при использовании данного лоскута могут определить новый стандарт для замещения малых и средних мягкотканых дефектов полости рта.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Reconstruction of defects following surgery for head and neck cancer possess challenges for surgeons. The choice of the reconstruction option is determined by various factors, namely: the size and location of the defect, patients’ age, concomitant pathology, prognosis of the disease, and individual decisions of surgeons and patients. Microvascular surgery is a highly successful and relatively safe method for the reconstruction of large head and neck defects. However, the use of revascularized autografts is not possible in all categories of patients, and therefore the search for new reconstruction techniques is necessary to improve functional, aesthetic results and reduce trauma.</p></sec><sec><title>Material and methods</title><p>Material and methods. The use of different types of facial artery mucous-muscular (FAMM) flaps for the reconstruction of oral cavity defects after tumor resection was analyzed. From June 2020 to march 2021, 7 patients with histologically verified t2–4N0–1 oral cancer were treated at Blokhin National medical Research center of oncology (Moscow). The patients underwent tumor resection with one-stage reconstruction using different types of FAMM flaps. All patients underwent adjuvant radiation therapy.</p></sec><sec><title>Results</title><p>Results. When cutting out the flap, an additional skin incision was not performed, the defect in the oral cavity was replaced by tissues of the same composition while preserving the small salivary glands, respectively, there was no hair growth and a defect in the donor bed. None of the cases had malnutrition of the flap.</p></sec><sec><title>Conclusion</title><p>Conclusion.
