В обзор включены данные рандомизированных клинических исследований, оценивающих различные диагностические алгоритмы выявления метастатического поражения костей, частоту метастазов в позвоночный столб и длинные кости в зависимости от гистологической структуры первичной опухоли, их количество, в соответствии с этим и варианты хирургического или лучевого лечения, опубликованные за последние 10 лет. Также оценивались различные прогностические факторы, на основании которых можно определить ожидаемую продолжительность жизни пациента.</p></sec><sec><title>Результаты</title><p>Результаты. Опубликованы диагностические алгоритмы и шкалы, которые помогают выбрать оптимальную тактику лечения с учетом факторов прогноза заболевания. Благодаря этому улучшаются результаты лечения больных с метастатическим поражением позвоночника и длинных костей за счет индивидуализированного выбора тактики и объема хирургического вмешательства.</p></sec><sec><title>Заключение</title><p>Заключение. Необходимо продолжать научные исследования для выявления новых прогностических факторов, влияющих на онкологический прогноз при метастатическом поражении костей. Это приведет к оптимизации методик хирургического лечения пациентов с метастазами в кости и разработке новых алгоритмов выбора объема хирургических вмешательств.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: to conducted a systematic analysis of data available in the current literature, modern diagnosticalgorithms, prognostic factors, survival and options for surgical treatment of metastases in the spine andlong bones of the extremities.</p></sec><sec><title>Material And Methods</title><p>Material And Methods. The review included data from randomized clinical trials evaluating various diagnostic algorithms for detecting bone metastases, the frequency of metastases to the vertebral column and long bones, their number, and options for surgical or radiation treatment published over the past 10 years. The review also evaluated various prognostic factors that can be used to determine the patient’s life expectancy.</p></sec><sec><title>Results</title><p>Results. Diagnostic algorithms and scales that help choose treatment strategy, taking into account prognostic factors, were published. The treatment outcomes in patients with spine and long bone metastases were shown to be improved due to personalized surgical treatment strategies.</p></sec><sec><title>Conclusion</title><p>Conclusion. Further studies are needed to identify new prognostic factors determining the oncological outcomes in patients with bone metastases.
