Актуальность. Гепатоидная аденокарцинома представляет собой карциному с железистой и гепатоцеллюлярной дифференцировкой. Данная разновидность опухоли легкого встречается крайне редко. По данным базы редких клинических наблюдений, опухоль обладает склонностью к раннему метастазированию, агрессивному течению заболевания. Лечебная тактика по ведению данной группы пациентов в настоящее время не выработана ввиду малого количества наблюдений. Наиболее эффективным способом является хирургический метод. В литературе описано всего 51 наблюдение данной патологии. В отечественной литературе настоящее наблюдение является вторым.

Описание

Описание. Представлен клинический случай гепатоидной аденокарциномы легкого у мужчины 54 лет, у которого при обследовании по поводу новообразования легкого выявлена гепатоидная аденокарцинома. Описаны сложности морфологической диагностики данного гистотипа опухоли, т.к. морфологическое сходство гепатоидной аденокарциномы и гепатоцеллюлярного рака требует комплексного подхода в дифференциальной диагностике данных патологий c целью постановки точного диагноза. Представлена тактика ведения пациента.

Заключение

Заключение. Гепатоидная аденокарцинома легкого представляет собой редкое по гистологической структуре злокачественное новообразование легкого, с неблагоприятным прогнозом, что продемонстировано в нашем наблюдении.

Hepatoid adenocarcinoma is a rare type of adenocarcinoma with hepatocellular differention, which is characterized by early metastatic spread and poor prognosis. The treatment strategy in not clearly defined due to an extreme rarity of the tumor. Surgery is the most effective tool as a part of multimodality concept. So far, only 51 cases have been described in the literature, and only one in Russia.

Clinical case description. We report a case of metastatic hepatoid adenocarcinoma of the lung in a 54-year-old male patient. The difficulties in morphologic differential diagnosis between adenocarcinoma of the lung and primary liver cancer were described. The strategy for managing patients with hepatoid adenocarcinoma of the lung was demonstrated.

Conclusion. Hepatoid adenocarcinoma of the lung is a rare tumor with unfavourable outcome.
