Материал и методы. В группу исследования включены 107 больных раком молочной железы (средний возраст - 57,2 ± 11,2 года) без неоадъювантной химиотерапии. Проводили морфологическое и иммуногистохимическое исследование образцов опухоли молочной железы, полученных в результате оперативного лечения. Гистологический тип рака молочной железы устанавливали согласно рекомендациям ВОЗ (The WHO Classification of Breast Tumours, 5th Edition, 2019). В исследование включали только случаи с инвазивной карциномой неспецифического типа. Оценку клинических параметров проводили на основании данных анализа медицинских карт пациентов. Статистический анализ проведен с помощью программы Statistica 10.0, различия считались значимыми при уровне статистической значимости р&lt;0,05. Результаты. Феномен краудинга в опухоли при раке молочной железы ассоциирован с лимфогенным метастазированием, при этом особое значение имеет внутриопухолевая гетерогенность (различные опухолевые структуры). Во всех случаях (n=40) с лимфогенными метастазами в солидных структурах морфологически зарегистрирован феномен краудинга, при этом в солидных структурах опухоли отмечено снижение уровня индекса пролиферативной активности Ki67. В участках опухоли с наличием краудинга в солидных структурах инфильтративного компонента при метастатическом поражении лимфатических узлов значимо реже регистрировалась позитивная экспрессия интегрина β1. Заключение. На основании результатов исследования высказывается предположение, что в основе механизма, инициирующего процесс опухолевой инвазии, могут лежать краудинг и базальная клеточная экструзия, при этом обнаруженные закономерности являются проявлением изменений свойств клеток с приобретением более высокого потенциала в отношении подвижности и клеточной миграции.</p></abstract><trans-abstract xml:lang="en"><p>Purpose: to evaluate the crowding-induced morphological changes in invasive breast carcinoma and compare the data obtained with the parameters of the tumor progression. Material and Methods. The study group included 107 patients (mean age: 57.2 ± 11.2 years) who did not receive neoadjuvant chemotherapy. Breast cancer morphological and immunohistochemical images were studied. The histological type of breast cancer was determined according to WHO recommendations (The WHO Classification of Breast Tumors, 5th Edition, 2019). The study included only cases with invasive breast carcinoma of no specific type. Clinical parameters were assessed using the patients' medical history. Statistical analysis was carried out using the Statistica 10.0 program. A p-value of 0.05 or lower was considered statistically significant. Results. The phenomenon of crowding in breast cancer was shown to be associated with lymph node metastases, with intratumoral heterogeneity of particular importance. The phenomenon of crowding was morphologically confirmed in all cases (n=40) with lymph node metastases in solid tumors. In these solid tumor structures, a decrease in the expression level of the proliferative index Ki67 was observed. In tumor sites with the evidence of crowding phenomenon in solid structures with lymph node metastasis, positive expression of integrin β1 was less frequently observed. Conclusion.
