Золотым стандартом лечения почечно-клеточного рака (ПКР) являются резекция почки или нефрэктомия в зависимости от стадии или локализации образования. Сохранение функции органа имеет большое значение для пациентов с локализованными опухолями, особенно при наличии единственной почки, предшествующей хронической болезни почек (ХБП), протеинурии или множественных/ билатеральных образований. Важной целью резекции почки является сохранение функциональной активности органа, однако данная операция в любом случае приводит к ее снижению вследствие потери массы васкуляризированных нефронов и необратимого ишемического повреждения. Описание клинического случая. Представлен клинический случай пациентки 24 лет с диагнозом: Рак правой почки cT1bN0M0, I стадия. По данным МСКТ органов брюшной полости и забрюшинного пространства с внутривенным контрастированием выявлено опухолевое образование правой почки, вдающееся в синус почки и вызывающее пиелокаликоэктазию. В связи со сложным анатомическим расположением опухоли, предполагающим длительное время ишемии почки, была выполнена лапароскопическая резекция правой почки в условиях селективной «внутриартериальной» холодовой ишемии по методике, разработанной в МНИОИ им. П.А. Герцена, которая была запатентована. Заключение. Холодовая артериальная перфузия имеет ряд преимуществ. Во-первых, она позволяет добиться быстрого снижения температуры почечной паренхимы до 25 °С, что обеспечивает более комфортную и быструю резекцию почки и ушивание дефекта. Во-вторых, она исключает обратный венозный ток крови благодаря положительному перфузионному давлению в собирательной системе. В-третьих, она не влияет на экспозицию интраоперационной картины, т. к. не предполагает наличие в рабочей зоне дополнительных объектов (например, лед или зажимы). Согласно данным литературы, в наиболее сложных случаях (резекция единственной почки, опухоль большого диаметра, высокий показатель по шкале R.E.N.A.L.) гипотермия является методом выбора.</p></abstract><trans-abstract xml:lang="en"><p>Background. Depending on the stage or location of the tumor, kidney resection or nephrectomy is the gold standard of treatment for renal cell carcinoma Preservation of organ function is essential for patients with localized tumors, especially with one kidney, chronic kidney disease, proteinuria, or multiple/bilateral masses. An important goal of kidney resection is to preserve the functional activity of the organ; however, the surgery leads to its decrease because of the loss of vascularized nephrons and irreversible ischemic damage. Description of the clinical case. A 24-year-old female was diagnosed with cT1bN0M0, Stage I right kidney cancer. An abdominal cavity and retroperitoneal MSCT with intravenous contrast showed a mass in the right kidney, extending into the renal sinus and causing pyelocalicoectasia. A laparoscopic resection of the right kidney was performed using the patented technique developed at the P.A. Herzen Moscow Oncology Research Institute and providing selective intra-arterial cold ischemia. This was prompted by the complex tumor anatomy, which indicated a prolonged kidney ischemia. Conclusion. Cold arterial perfusion has several advantages. First, it quickly cools the renal parenchyma to 25 °C, enabling faster and more comfortable kidney resection and suturing. Second, it eliminates the reverse venous blood fow caused by the positive perfusion pressure in the collecting system. Third, it does not imply the additional objects (for example, ice or clamps) in the work area, therefore not affecting the intraoperative view.
