Представлены результаты оценки диагностической эффективности использования уточняющей эндоскопической методики – аутофлуоресценции (AFI) у 101 пациента с патологией гортани и гортаноглотки: I группа – 53 больных с хроническими гиперпластическими заболеваниями, ii группа – 48 больных раком гортани или гортаноглотки.</p></sec><sec><title>Результаты</title><p>Результаты. Включение в алгоритм обследования пациентов с хронической патологией верхних дыхательных путей видеоларингоскопии (ВЛС) с режимом AFI позволило значимо улучшить показатели диагностической эффективности эндоскопического исследования у больных с предраковыми изменениями (дисплазия high grade) слизистой оболочки гортани. При этом чувствительность комплексной ВЛС составила 85,7 %, специфичность – 96,9 %, точность – 92,5 %, тогда как чувствительность видеоларингоскопии в белом свете равнялась 50,0 %, специфичность – 96,0 %, точность – 71,7 % (p&lt;0,05). Комплексная видеоларингоскопия с методикой AFI позволяет не только выявить патологические изменения на слизистой оболочке гортани и гортаноглотки, но и определить их злокачественный потенциал, а также определить участки для таргетной биопсии c диагностическими трудностями. При стандартной ВЛС в белом свете у 9 (18,8 %) больных раком гортани и гортаноглотки отмечены трудности в дифференциальной диагностике рака гортани (n=4) и при оценке истинной распространенности рака гортаноглотки (n=5). В 3 из 4 диагностически сложных случаев применение режима AFI при комплексной ВЛc позволило подтвердить диагноз рака гортани и в 5 случаях определить истинные границы рака гортаноглотки.</p></sec><sec><title>Заключение</title><p>Заключение. Дополнение стандартной видеоларингоскопии высокотехнологичной методикой аутофлуоресцентной эндоскопии позволяет с высокой точностью диагностировать предопухолевые изменения и злокачественные новообразования гортани и гортаноглотки.</p></sec></abstract><trans-abstract xml:lang="en"><p>The purpose of the study was to assess the diagnostic value of endoscopic autofluorescence imaging (AFI) in patients with premalignant lesions and primary laryngeal/hypopharyngeal cancer.</p><sec><title>Material and Methods</title><p>Material and Methods. The diagnostic value of AFI was assessed in 53 patients with chronic hyperplastic laryngeal/hypopharyngeal lesions and 48 patients with laryngeal/hypopharyngeal cancer.</p></sec><sec><title>Results</title><p>Results. The inclusion of video laryngoscopy with AFI in the algorithm for examining patients with chronic diseases of the upper respiratory tract made it possible to significantly improve the diagnostic efficacy of endoscopic examination in patients with premalignant lesions (high grade dysplasia) of the laryngeal mucosa. Video laryngoscopy with AFI was found to achieve higher sensitivity, specificity and accuracy rates than white light video laryngoscopy (87.5, 96.9 and 92.5 vs 50.0, 96.0 and 71.7 %, respectively, p&lt;0.05). The video laryngoscopy with AFI allowed identification of not only pathological changes in the laryngeal and hypopharyngeal mucosa but also their malignant potential, as well as identification of areas for targeted biopsy. In 9 (18.8 %) laryngeal cancer patients, the standard white light video laryngoscopy demonstrated difficulties in the differential diagnosis of laryngeal cancer (n=4) and in assessing the extent of laryngeal cancer (n=5). In 3 out of 4 diagnostically difficult cases, video laryngoscopy with AFI made it possible to confirm the diagnosis of laryngeal cancer and in 5 cases to accurately determine the extent of hypopharyngeal cancer.</p></sec><sec><title>Conclusion</title><p>Conclusion.
