Трижды негативный рак молочной железы является наиболее агрессивным молекулярным подтипом среди злокачественных опухолей молочной железы. Из-за высокой пролиферативной активности опухолевых клеток и отсутствия мишеней для таргетной терапии основным методом в комплексном лечении данного подтипа является своевременное и обоснованное назначение неоадъювантной химиотерапии. Оптимальное время начала неоадъювантной химиотерапии определяется как менее 8 нед, превышение этого периода может влиять на эффективность лечения, а также приводить к местному или системному прогрессированию опухолевого процесса. Достижение полного патоморфологического регресса на фоне неоадъювантной химиотерапии обеспечивает снижение относительного риска рецидива более чем на 70 %, что, в свою очередь, ведет к значимому улучшению долгосрочной выживаемости данной когорты пациентов. Описание клинического случая. Пациентка 32 лет обратилась в клинику спустя 15 мес после постановки диагноза трижды негативного рака молочной железы. На протяжении 15 мес пациентка лечилась методами альтернативной медицины, и на момент обращения зарегистрирован выраженный местный рост опухоли с массивным распадом, симптомами опухолевой интоксикации и нутритивных нарушений. На фоне симптоматической и поддерживающей терапии проведена неоадъювантная химиотерапия: 12 циклов паклитаксел + карбоплатин в еженедельном режиме с последующими 4 циклами доксорубицин + циклофосфамид в дозоуплотненном режиме. На фоне лечения отмечается выраженная положительная динамика в виде значительного уменьшения размера опухоли, улучшения общего состояния больной. Клинически значимых побочных эффектов, потребовавших госпитализации или редукции дозы препарата, не наблюдалось. По завершении неоадъювантного этапа лечения пациентке выполнена радикальная мастэктомия по Madden слева. По результатам гистологического исследования послеоперационного материала достигнут полный патоморфологический регресс первичной опухоли и регионарных лимфоузлов. В адъювантном режиме назначена дистанционная лучевая терапия. Спустя 3 года после постановки диагноза и 1,5 года после завершения комплексного лечения пациентка продолжает жить полноценной жизнью и планирует отсроченную реконструкцию удаленной левой молочной железы с аугментационной маммопластикой справа. Заключение. Данный клинический случай демонстрирует редкий вариант массивного местного прогрессирования трижды негативного рака молочной железы с полным патоморфологическим регрессом после неоадъювантной химиотерапии спустя 15 мес после постановки диагноза.</p></abstract><trans-abstract xml:lang="en"><p>Background. Triple-negative breast cancer is the most aggressive molecular subtype among breast malignancies. Due to the high proliferative activity of tumor cells and the lack of targets for targeted therapy, neoadjuvant chemotherapy plays the main role in complex treatment of this subtype. The optimal time to start neoadjuvant chemotherapy is defined as less than 8 weeks. Delays in time to treatment initiation may adversely affect treatment outcomes, as well as cause local or systemic disease progression. Achieving complete pathological response after neoadjuvant chemotherapy reduces the relative risk of relapse by more than 70 %, which, in turn, leads to a significant improvement in long-term survival of these patients. Description of the clinical case. A 32-year-old woman came to the clinic 15 months after being diagnosed with triple-negative breast cancer. The patient was treated with alternative medicine methods for 15 months, and upon admission to the hospital, the patient had pronounced local tumor growth with massive decay, tumor lysis syndrome and nutritional disorders. We decided to perform neoadjuvant chemotherapy: 12 cycles of weekly paclitaxel + carboplatin, followed by 4 cycles of dose-dense doxorubicin + cyclophosphamide. Chemotherapy resulted in a significant reduction in tumor size and improvement of health status of the patient. There were no clinically significant side effects requiring hospitalization or dose reduction. Upon completion of neoadjuvant treatment, the patient underwent mastectomy. The histological examination revealed a complete pathological response of the primary tumor and regional lymph nodes. The patient underwent adjuvant external beam radiation therapy. The patient is alive 3 years after diagnosis and 1.5 years after treatment completion. She plans delayed breast reconstruction with contralateral breast augmentation. Conclusion.
