Согласно общемировым данным, рак желудка (РЖ) занимает 5-е место по частоте встречаемости среди злокачественных онкологических заболеваний с сохраняющимися высокими показателями онкоспецифической летальности. Однако в последние десятилетия отмечена тенденция к росту заболеваемости РЖ среди пациентов молодого возраста (от 18 до 40 лет), и на данный момент, по оценкам исследователей, этот показатель достигает 4,4–9,8 %. Цель исследования – оценить эффективность хирургического, комбинированного и паллиативного лечения у больных РЖ молодого возраста (18–45 лет). Материал и методы. В ретроспективное исследование включено 129 больных молодого возраста, оперированных в объемах радикального, циторедуктивного и паллиативного вмешательств в сочетании (или без) с вариантной лекарственной терапией по поводу локализованного, местнораспространенного и первично-диссеминированного рака желудка. Больные были разделены на 3 группы: 1) хирургическую группу составили 27 пациентов, которым выполнено хирургическое лечение без предшествующей или последующей лекарственной терапии; 2) в группу комбинированного (хирургия + ХТ) лечения включены 58 больных, которым выполняли радикальные, а также циторедуктивные операции (при индексе перитонеального канцероматоза менее 7 баллов) в объеме СС0 в сочетании с вариантной полихимиотерапией (неоадъювантной, адъювантной, периоперационной, одномоментно, либо с гипертермической интраоперационной внутриполостной химиотерапией (ГИВХ), а с ГИВХ в изолированном варианте); 3) в дополнительную (паллиативную) группу включено 44 пациента, у которых при стадирующей лапароскопии в начале лечения был верифицирован синхронный канцероматоз с индексом перитонеальной диссеминации более 7 баллов (PCI >7 баллов), и получавших лечение по программе системной лекарственной химиотерапии в сочетании с сеансами внутрибрюшной аэрозольной химиотерапии под давлением (ВАХД). Результаты. В группе хирургического лечения медиана общей выживаемости (ОВ) составила 58 мес. Медиана ОВ в комбинированной (хирургия + ХТ) и дополнительной (паллиативная) группах лечения достигла 30 и 13 мес соответственно. Сопоставление комплексного лечения IV стадии в комбинированной и дополнительной группах, в которых проведена полная циторедукция с вариантной ПХТ и ГИВХ, в сравнении с ВАХД показывает незначительное превалирование по медиане ОВ, составившей 17 и 13 мес соответственно. Заключение. Агрессивное мультимодальное лечение в связи с отсутствием выраженного коморбидного статуса у молодых пациентов с РЖ позволит интенсифицировать периоперационную лекарственную терапию и выполнить расширенные мультивисцеральные вмешательства, способствующие радикальности хирургического лечения для улучшения показателей медиан БРВ и ОВ.

Introduction. According to global data, gastric cancer (GC) is the 5-th most common malignancy with a high cancer-related mortality rate. However, in recent decades, there has been a tendency towards an increase in the incidence of GC among young patients (18 to 40 years old), which currently amounts to 4.4–9.8%. Aim: to evaluate the effectiveness of surgical, combined and palliative treatment options in early-onset GC. Material and Methods. the study included 129 patients aged 18–45 years, who underwent radical, cytoreductive and palliative surgery with or without combination with drug therapy for localized, locally-advanced and primary disseminated GC. the patients were divided into three clinical groups: 1) the surgical group (n=27) included patients with only surgical treatment; 2) the group of combined treatment (n= 58) included patients with PCI <7 who underwent surgery with the volume of CC0 in combination with CT (neoadjuvant, adjuvant, perioperative, simultaneously with or without hyperthermic intraoperative intraperitoneal chemotherapy (HIPEC) and patients who underwent only HIPEC; 3) the additional (palliative) group (n=44) included patients with PCI>7 who underwent systemic CT in combination with pressurized intraperitoneal aerosol chemotherapy (PIPAC). Results. the median overall survival (OS) in three groups was 58, 30 and 13 months, respectively. In patients with IV stage of disease who underwent HIPEC with CC0 surgery, OS in the 2nd group was 17 in comparison with 13 months in the third group (PIPEC). Conclusion. Aggressive multimodal treatment in the absence of comorbid status in young patients will allow for intensification of both the perioperative drug therapy component and the implementation of extended multivisceral resections that facilitate radical surgical treatment to improve both DFS and OS.
