Обширное лимфогенное метастазирование поражает отдаленные регионы лимфоузлов и серозные оболочки. Практически отсутствуют сведения о массивном метастатическом поражении паренхиматозных органов в результате опухолевой эмболии их лимфатических сосудов. Данный случай демонстрирует именно такое распространение опухолевого процесса. Цель исследования – демонстрация случая летального исхода при раке желудка с генерализованным лимфогенным метастатическим поражением паренхиматозных органов. Описание случая летального исхода. Представлен случай летального исхода с обширным лимфогенным метастазированием рака желудка с поражением паренхиматозных органов. Пациентка, 56 лет, получала симптоматическое лечение в течение 11 койко-дней в стационаре общехирургического профиля с диагнозом: Рак желудка, IV стадия. Состоявшееся желудочное кровотечение. Во время патологоанатомического исследования обнаружена обширная лимфоваскулярная инвазия с массивной эмболией лимфатических сосудов брюшины, плевры, перикарда, забрюшинной клетчатки и практически всех органов за исключением головного мозга. Опухолевые эмболы в лимфатических сосудах интенсивно экспрессировали субъединицы интегринов a6 и b4, а также субъединицу ламинина γ2. Заключение. Обширное метастатическое поражение паренхиматозных органов может быть результатом опухолевой эмболии лимфатических сосудов. Данный вариант прогрессирования может быть ассоциирован с экспрессией опухолевыми клетками субъединиц интегринов α6 и β4, а также субъединицы ламинина γ2.</p></abstract><trans-abstract xml:lang="en"><p>Generalized metastases to parenchymatous organs in gastric cancer are usually associated with hematogenous spread of cancer cells. Generalized lymphogenous metastasis involves the spread of cancer cells to distant lymph nodes and serous membranes. There is very little information on generalized metastases to parenchymatous organs due to tumor embolism of their lymphatic vessels. Objective: to present a fatal case with generalized lymphogenous metastases to parenchymatous organs. Case presentation. A 56-year-old female patient with gastric cancer presented to the hospital with generalized lymphogenous metastases to parenchymatous organs. The patient received palliative treatment in the hospital for 11 days. The pathological examination showed extensive lymphovascular invasion with massive embolism of the lymphatic vessels of the peritoneum, pleura, pericardium, retroperitoneal tissue and almost all organs except the brain. Tumor emboli in the lymphatic vessels intensively expressed integrin subunits a6 and b4, as well as the laminin subunit γ2. Conclusion. Widespread metastases to parenchymatous organs may be caused by tumor embolism of the lymphatic vessels.
