В одноцентровом сравнительном исследовании с ретроспективным анализом проспективно сформированных клинических данных проанализированы результаты обследования и лечения 60 пациентов с неоперабельным МРРПЖ llb-lll стадии в сопоставимых группах: все пациенты получили химиолучевое лечение, включавшее 3 и более цикла полихимиотерапии (ПХТ) по схеме FOLFlRlNOX или GEMCAP в сочетании с лучевой терапией (ЛТ) в режиме дневного дробления дозы (группа А) или со стереотаксической лучевой терапией (СТЛТ) (группа В).</p></sec><sec><title>Результаты</title><p>Результаты. Получены данные о высокой эффективности применения химиолучевой терапии в режиме дневного дробления дозы в сравнении с химиолучевой терапией в режиме стереотаксической ЛТ. Ранние лучевые реакции соответствовали l-ll степени и наблюдались в обеих группах. Наиболее часто регистрировали повышение уровня трансаминаз: у 36,7 % пациентов в группе А и у 46,7 % - в группе В (p=0,59). Тошноту/рвоту наблюдали у 23,3 и 40,0 % соответственно (p=0,18). Анализ непосредственных результатов показал, что общая частота объективного ответа была значимо выше в группе А по сравнению с группой B (93,3 vs 70,0 %; p=0,03). В то же время в группе B значимо чаще отмечали прогрессирование заболевания (30,0 vs 6,7 %; p=0,042). Одногодичная общая выживаемость (ОВ) больных при ЛТ в режиме дневного дробления дозы составила 73 %, в группе стереотаксической ЛТ - 48 % (p&lt;0,05). Двухгодичная общая выживаемость больных при ЛТ в режиме дневного дробления дозы в сравнении с СТЛТ - 39 vs 15 % (p&lt;0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Продемонстрировано, что применение комбинированной терапии у тщательно отобранной группы пациентов с неоперабельным МРРПЖ llb-lll стадии позволяет увеличить показатели средней продолжительности жизни, общей выживаемости, частоту объективного ответа. Совершенствование техники лучевого лечения способствует снижению частоты ранних общих лучевых реакций. Методика дневного дробления дозы является более эффективной и может рассматриваться как предпочтительный режим ЛТ при лечении неоперабельного МРРПЖ.</p></sec></abstract><trans-abstract xml:lang="en"><p>The purpose of the study was to improve radiotherapy efficacy by optimizing fractionation regimens in patients with inoperable stage llb-lll locally advanced pancreatic cancer.</p><sec><title>Material and Methods</title><p>Material and Methods. Treatment outcomes of 60 patients with inoperable stage llb-lll locally advanced pancreatic cancer were analyzed in a single-center comparative study with a retrospective analysis of prospectively collected clinical data. All patients received chemoradiotherapy, including 3 or more cycles of polychemotherapy (PCT) with FOLFlRlNOX or GEMCAP regimen in combination with daily split-dose radiation therapy (RT) (group A) or stereotactic radiotherapy (STRT) (group B).</p></sec><sec><title>Results</title><p>Results. The efficacy of daily split-dose RT was found to be higher than that of stereotactic RT Early radiation reactions corresponded to grade l-ll and were observed in both groups. lncreased levels of transaminases were the most common: in 36.7 % of patients in group A and in 46.7 % in group B (p=0.59). Nausea/vomiting was observed in 23.3 % and 40.0 %, respectively (p=0.18). An analysis of the immediate results showed that the overall rate of objective response was significantly higher in group A compared with group B (93.3 vs 70.0 %; p=0.03). However, disease progression was observed more frequently in group B than in group A patients (30.0 vs 6.7 %; p=0.042). The daily split-course regimen demonstrated significantly higher one-year (73 vs 48 %) and two-year (39 vs 15 %) overall survival rates than stereotactic RT (p&lt;0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion. Our study demonstrated the use of combination therapy in a carefully selected group of patients with inoperable stage llb-lll locally advanced pancreatic cancer. This type of therapy allows for increased life expectancy, overall survival, and objective response rates. Moreover, continuous improvements in radiation therapy techniques help reduce the incidence of early systemic radiation reactions.
