В ретроспективное одноцентровое исследование включены 16 пациентов с мКРРПЖ, получивших не менее 4 инъекций Ra-223 с последующей терапией 177Lu-ПСМА (не менее 2 циклов) в 2022-26 гг Медиана возраста составила 69,5 года. Все больные имели костные метастазы, 94 % ранее получали доцетаксел. Оценивались биохимический ответ (снижение простатспецифического антигена (ПСА)&gt;50 %), радиологический ответ по данным ПЭТ/КТ с ПСМА и остеосцинтиграфии, а также токсичность по CTCAE v5.0.</p></sec><sec><title>Результаты</title><p>Результаты. Полный курс Ra-223 (6 инъекций) получили 12 пациентов. Биохимический ответ на Ra-223 отмечен у 2 (12,5 %), радиологический - у 5 (31,3 %) пациентов. При медиане интервала 2,2 мес начата терапия 177Lu-ПСМА (медиана 5 циклов). Биохимический ответ на 177Lu-ПСМА достигнут у 11 (68,8 %) больных, в том числе у 7 из 9 с прогрессированием на Ra-223. Радиологический ответ зафиксирован у 9 (56,3 %). Медиана ПСА снизилась со 130,5 до 15,9 нг/мл. Тяжелая (3 степени) гематологическая токсичность наблюдалась у 2 (12,5 %) пациентов, ксеростомия - у 3 (18,8 %).</p></sec><sec><title>Заключение</title><p>Заключение. Последовательное применение Ra-223 и 177Lu-ПСМА демонстрирует высокую эффективность (68,8 % биохимического ответа) и приемлемый профиль безопасности у пациентов с мКРРПЖ, включая случаи прогрессирования на фоне терапии Ra-223, что обосновывает целесообразность данной стратегии в клинической практике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to evaluate the efficacy and safety of sequential treatment with radium-223 chloride (Ra-223) followed by 177Lu-PSMA in patients with metastatic castration-resistant prostate cancer (mCRPC) in real-world clinical practice.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. This retrospective single-center study included 16 mCRPC patients who received at least 4 Ra-223 injections followed by 177Lu-PSMA therapy (at least 2 cycles) between 2022 and 2026. The median age was 69.5 years. All patients had bone metastases; 94 % had prior docetaxel treatment. We assessed biochemical response (prostate-specific antigen (PSA) decline &gt;50 %), radiological response using PSMA PET/CT and bone scintigraphy, and toxicity according to CTCAE v5.0.</p></sec><sec><title>Results</title><p>Results. Twelve patients received a full course of Ra-223 (6 injections). Biochemical response to Ra-223 was observed in 2 (12.5 %) patients, radiological response in 5 (31.3 %). After a median interval of 2.2 months, 177Lu-PSMA therapy was initiated (median 5 cycles). Biochemical response to 177Lu-PSMA was achieved in 11 (68.8 %) patients, including 7 out of 9 who had progressed on Ra-223. Radiological response was recorded in 9 (56.3 %) patients. Median PSA level decreased from 130.5 to 15.9 ng/mL. Severe (grade 3) hematological toxicity occurred in 2 (12.5 %) patients, xerostomia in 3 (18.8 %).</p></sec><sec><title>Conclusion</title><p>Conclusion.
