Материал и методы. Представлен клинический случай отсроченной БСЛУ после ранее выполненной подкожной мастэктомии с сохранением САК и одномоментной имплантат-ассоциированной реконструкции молочной железы. Работа носит описательный характер и не предназначена для оценки диагностической точности, чувствительности, специфичности или частоты ложноотрицательных результатов методики. Пациентке 40 лет, ранее перенесшей подкожную мастэктомию справа с сохранением САК и одномоментной реконструкцией молочной железы имплантатом, была выполнена отсроченная биопсия сторожевых лимфатических узлов для стадирования регионарного лимфатического коллектора. Предоперационное картирование проводили путем периареолярного подкожного введения 99mTc-Nanocoll активностью 50 МБк с последующей лимфосцинтиграфией и ОФЭКТ-КТ. В день операции ICG вводили внутрикожно в четырех периареолярных точках. Интраоперационную идентификацию сторожевых лимфатических узлов выполняли с использованием системы флуоресцентной визуализации в ближнем инфракрасном диапазоне MARS, версия 2.0.0.7, 2024, и ручного гамма-зонда. Первичной процедурной конечной точкой являлся технический успех, определяемый как интраоперационная идентификация и удаление, как минимум, одного сторожевого лимфатического узла. Результаты. По данным лимфосцинтиграфии и ОФЭКТ-КТ выявлено накопление радиофармпрепарата в двух правых подмышечных лимфатических узлах. Интраоперационно оба сторожевых лимфатических узла были успешно идентифицированы и удалены с применением комбинированной гамма-навигации и флуоресцентной визуализации. В обоих удаленных лимфатических узлах отмечалось совпадение радиоактивного и флуоресцентного сигналов. Метастатического поражения сторожевых лимфатических узлов при гистологическом исследовании не выявлено. Интраоперационных и ранних послеоперационных осложнений не зарегистрировано. Имплантат, кожные лоскуты после мастэктомии и САК оставались жизнеспособными; признаков ишемии, инфекции, экспозиции имплантата или несостоятельности реконструкции не отмечено. Заключение. В представленном клиническом случае отсроченная БСЛУ с применением двойной трассировки была технически успешно выполнена и не сопровождалась ранними периоперационными осложнениями. Полученные данные свидетельствуют о технической выполнимости процедуры в данной клинической ситуации, однако не позволяют оценить диагностическую точность, воспроизводимость или онкологическую безопасность метода.</p></abstract><trans-abstract xml:lang="en"><p>Objective: to present a clinical case of delayed dual-tracer sentinel lymph node biopsy using technetium-99m radiocolloid and indocyanine green fluorescence in a patient who previously underwent nipple-sparing mastectomy with immediate implant-based breast reconstruction, and to analyze the technical feasibility and early perioperative safety of the procedure. Material and Methods. This article presents a clinical case of delayed sentinel lymph node biopsy after previous nipple-sparing mastectomy with immediate implant-based breast reconstruction. The report is descriptive and was not intended to assess diagnostic accuracy, sensitivity, specificity, or the false-negative rate of the technique. A 40-year-old woman with a history of right nipple-sparing mastectomy and immediate implant-based reconstruction underwent delayed axillary sentinel lymph node biopsy for staging regional lymphatic basin. Preoperative lymphatic mapping was performed using periareolar subcutaneous injection of 50 MBq 99mTc-Nanocoll, followed by lymphoscintigraphy and SPECT-CT. On the day of surgery, indocyanine green was injected intradermally at four periareolar points. intraoperative detection was performed using a near-infrared fluorescence imaging system, MARS, version 2.0.0.7, 2024, and a handheld gamma probe. The primary procedural endpoint was technical success, defined as intraoperative identification and excision of at least one sentinel lymph node. Results. Preoperative lymphoscintigraphy and SPECT-CT demonstrated radiotracer uptake in two right axillary lymph nodes. Intraoperatively, both sentinel lymph nodes were successfully identified and excised using combined gamma-probe detection and fluorescence guidance. Concordance between radioactive and fluorescent signals was observed in both removed nodes. Histological examination showed no metastatic involvement. No intraoperative or early postoperative complications were recorded. The implant, mastectomy flaps and nipple-areolar complex remained viable, with no evidence of ischemia, infection, implant exposure or reconstructive failure. Conclusion. In this clinical case, delayed dual-tracer sentinel lymph node biopsy was technically successful and was not associated with early perioperative complications.
