В соответствии с действующими критериями отбора пациенты с метастатически пораженными контралатеральными лимфатическими узлами (корень легкого, аортальное окно, паратрахеальная зона) нуждаются в химиоили химиолучевой терапии с последующей оценкой динамики процесса и принятием решения о дальнейшем лечении. Данная группа пациентов считается потенциально неоперабельной. Технические сложности выполнения операций и реанимационно-анестезиологических манипуляций ставят вмешательства с резекцией бифуркации трахеи, бронхов и сосудов в разряд сложнейших. Подобные операции должны проводиться у тщательно отобранных пациентов и только в центрах, имеющих большой опыт подобных операций, что позволяет снизить количество интраи послеоперационных осложнений до минимума.</p></abstract><trans-abstract xml:lang="en"><p>Over the past few years, significant advances in surgical and anesthetic techniques as well as appropriate selection of patients have led to an improvement in the immediate and long-term treatment outcomes in patients with non-small cell lung cancer with involvement of tracheal bifurcation. In accordance with the current selection criteria, patients with contralateral lymph node metastases (lung root, aortic window, paratracheal area) require chemotherapy or chemoradiotherapy with subsequent estimation of follow-up and treatment strategy. Surgeries with resection of tracheobronchial bifurcation are considered technically complicated, and they should be performed in carefully selected lung cancer patients and only in specialized centers with extensive experience.
