Персонализированный подход предполагает индивидуальный выбор лекарственных средств и их доз для каждого конкретного пациента, обеспечивая максимально эффективную и безопасную фармакотерапию.</p><p>Цель исследования - анализ частот полиморфизмов UGT1A1 и DPYD генов и сопоставление данных генотипирования с иринотекан- и 5-фторурацил-индуцированной токсичностью.</p></sec><sec><title>Материал и методы</title><p>Материал и методы: использовали венозную кровь 94 пациентов европеоидного типа (46 мужчин и 48 женщин, медиана возраста - 61 год). Методом пиросеквенирования идентифицировали аллели *6 и *28 UGT1A1, методом ПЦР-РВ - аллель *2А DPYD.</p></sec><sec><title>Результаты</title><p>Результаты. Генотипирование 94 пациентов с онкопатологией толстой кишки не выявило мутации *2А в гене DPYD. Частота встречаемости аллелей *6 и *28 гена UGT1A1 составила 0,346 и 0,016 соответственно. У 24 % пациентов, схемы химиотерапии которых включали 5-фторурацил, были зафиксированы токсические эффекты со стороны кроветворной системы и желудочно-кишечного тракта. Развитие гематологических и негематологических токсических реакций было отмечено соответственно у 48 % и 50 % пациентов, получавших лечение иринотеканом. Билирубинемия ассоциировалась с генотипом *28/*28 UGT1A1. При этом наличие генотипа высокого риска (*28/*1, *28/*28 гена UGT1A1) достоверно коррелировало с развитием токсических эффектов химиотерапии (p=0,040).</p></sec><sec><title>Заключение</title><p>Заключение. Отсутствие носителей аллеля *2А DPYD в выборке со значительной долей выраженных нежелательных токсических реакций на 5-фторурацил пациентов Юга России обусловливает необходимость включения новых полиморфизмов гена DPYD в фармакогенетическое тестирование. Введение генотипирования полиморфизмов UGT1A1 в комплекс предварительного обследования целесообразно при планировании лечения иринотеканом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The personalized approach implies an individual choice of medicines and their doses for the patient, providing the most effective and safe pharmacotherapy. Objective: analysis of the frequencies of UGT1A1 and DPYD polymorphisms and comparison of genotyping data with irinotecan and 5-fluorouracil-induced toxicity, respectively.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. Venous blood of 94 Caucasian patients (46 men and 48 women, median age 61 years). The *6 and *28 UGT1A1 alleles were identified by pyrosequencing, and the *2А DPYD allele was identified by Real-time PCR.</p></sec><sec><title>Results</title><p>Results. The genotyping of 94 patients with colon cancer did not reveal the *2A SNP in the DPYD gene. The frequency rate of the *6 and *28 alleles of the UGT1A1 gene was 0.346 and 0.016, respectively. 24 % of patients receiving chemotherapy with 5-fluorouracil developed side effects associated with the circulatory system and the gastrointestinal tract. Hematological and nonhematological toxic reactions were noted in 48 % and 50 % of patients receiving irinotecan. Severe bilirubinemia was associated with the *28/*28 UGT1A1 genotype. The presence of a high-risk genotype (*28/*1, *28/*28 UGT1A1) correlated with the development of side effects (p=0.040).</p></sec><sec><title>Conclusion</title><p>Conclusion. The absence of carriers of the *2А DPYD allele in the sample with a significant proportion of pronounced adverse toxic reactions to 5-fluorouracil causes the need for the inclusion of new polymorphisms of the DPYD gene in pharmacogenetic testing.
