СОВРЕМЕННЫЕ СХЕМЫ ЛУЧЕВОЙ ТЕРАПИИ И БИОМАРКЕРЫ РАДИОРЕЗИСТЕНТНОСТИ ОПУХОЛЕВЫХ КЛЕТОК ПРЯМОЙ КИШКИ

Аннотация

Цель исследованияобобщение имеющихся данных о различных современных схемах (протоколах) лучевой терапии и их эффективности, а также известных в настоящее время маркерах радиорезистентности клеток. Материал и методы. Поиск соответствующих источников производился в системах PubMed и cochrane Library, анализировались публикации с 1984 по 2019 г., 60 из которых были  использованы для написания данного обзора. Результаты. Рак прямой кишки является одной из наиболее распространенных форм злокачественных новообразований, и за последнее десятилетие отмечен рост заболеваемости в экономически развитых странах всего мира. В России рак прямой кишки занимает одну из ведущих позиций и в настоящее время по числу диагностированных случаев и числу умерших пациентов уступает лишь таким заболеваниям, как рак легкого, рак желудка и рак молочной железы. Основным эффективным методом лечения рака прямой кишки, признанным как во всем мире, так и в России, остается выполнение предоперационной лучевой терапии с последующим хирургическим вмешательством. Однако на практике имеются случаи отсутствия реакции у пациентов на предоперационную лучевую терапию, что связано с радиорезистентностью опухолевых клеток. В настоящее время разработаны различные схемы лучевой терапии, отличающиеся по частоте возникновения местных рецидивов, общей выживаемости и качеству жизни больных. Также известны и маркеры радиорезистентности клеток – генетические (изменение экспрессии генов и микро-РНК, мутации) и протеомные (изменение экспрессии белков), которые могут не только нести прогностическую ценность, но также использоваться в качестве мишеней при терапии. Заключение. Таким образом, основная проблема, в которой заключается низкий ответ опухолей прямой кишки на различные схемы лучевой терапии, – радиорезистентность опухолевых клеток. И хотя ряд молекулярных биомаркеров был предложен в качестве предикторов ответа на лучевую терапию, на данный момент ни один из них не вошел в клиническую практику. Для создания эффективной панели молекулярных биомаркеров и улучшения персонифицированной помощи при раке прямой кишки требуются дальнейшие исследования.
