Роль жидкостной биопсии в диагностике прогрессирования глиобластомы

Аннотация

Цель исследования – обобщение имеющихся данных о диагностической ценности различных циркулирующих биомаркеров для диагностики рецидива глиобластомы.

Материал и методы. Проведен биоинформациооный поиск в универсальной базе данных PubMED, в базе данных экзосомальных белков, РНК и липидов ExoCarta, в базе данных внеклеточной ДНК, в базе данных внеклеточной ДНК SILVA.

Результаты. Мультиформная глиобластома (МГБ) – наиболее распространенная глиальная опухоль у взрослых с плачевным прогнозом. Лечение рецидива опухоли может улучшить выживаемость пациентов. Нейровизуализация – стандартный метод диагностики рецидива опухоли головного мозга. Однако методы нейровизуализации, позволяющие четко различать прогрессирование опухоли и связанные с лечением рентгенографические изменения на ранних стадиях, сегодня не найдены. Современное молекулярное профилирование опухоли в значительной степени зависит от резекции ткани или биопсии. Профилирование ткани имеет несколько недостатков при опухолях центральной нервной системы, в т. ч. проблемы, связанные с инвазивной биопсией, неоднородной природой многих злокачественных новообразований, когда небольшая биопсия может недооценивать мутационный профиль. Жидкая биопсия – перспективное направление современной онкологии. Сбор крови – это простая, малоинвазивная процедура, но спинномозговая жидкость позволяет более уверенно обнаруживать опухолевые маркеры, однако получение ее является сложной и инвазивной процедурой, которая может сопровождаться серьезными осложнениями.

Заключение. Маркеры биологических жидкостей, такие как циркулирующие опухолевые клетки, внеклеточные везикулы, внеклеточная ДНК и внеклеточная РНК, позволяют выявить МГБ, определить молекулярно-генетические особенности рака во время ответа на терапию и обнаружить рецидив МГБ на ранней стадии.
